How To Select Compression Garments for the Patient

To avoid backorders and other delays, we always recommend giving our Case Managers the flexibility to order from the manufacturer who can deliver the fastest.  You’ll see on our referral forms (below) that we use velcro wraps and wound-care appropriate compression stockings.  You always have the option to specify the garments you prefer.

How To Measure the Patient

How To Submit Your Referral

Here is the wound care referral form you’ll need.  We recommend downloading this first, and then using it to capture all the measurements.

Next, please submit your referral with all of the following:

  1. Referral Form
  2. Patient Demographics
  3. Copy of Insurance Card(s) — Front and Back
  4. Clinical Notes

Please specify whether we’re shipping to the patient’s home, or your clinic address.
